The Powassan & Area Family Health Team is proud to serve the patients in our catchment area. We are an interdisciplinary team of health care providers including a Nurse Practitioner,  Registered Practical Nurses, and Mental Health Clinicians in addition to our affiliated family physicians. All of our providers work together to offer a truly collaborative, team approach to primary care.

Our Family Health Team has 4 clinic sites: Powassan Clinic, Astorville: Docs on the Lake Family Practice, South River: South River Machar Medical Centre and Bonfield: Bonfield Medical Clinic. 

The Powassan & Area Family Health Team provides a wide range of primary care services and also develops and implements evidence-based, collaborative chronic disease management programs, health promotion and a thorough preventative care program. Our team utilizes an EMR (electronic medical record) and is always striving to incorporate more efficiency into our practice through IT solutions and integration.
