Our FHT staff work collaboratively with each other and our affiliated physicians to provide you with comprehensive primary care services.  Different providers on our team are able to provide different kinds of health services.

The Diabetes program aims to improve the care you receive for diabetes. The program includes screening to identify individuals who have diabetes, provides regular follow-up visits and helps individuals with diabetes to better manage their own care. Improvements in screening and self-management will help provide a reduction in blood pressure, cholesterol and blood sugar that can significantly reduce the risk of heart attack, kidney disease, eye damage and death.

You must be a patient of one of our primary care providers to access our Diabetes program.

Hypertension, also known as high blood pressure, affects one in five Canadians and is often called the silent killer because you may not know it is there. The Powassan & Area Family Health Team has developed a hypertension screening program based on best practice guidelines to annually screen patients so that we can identify whether or not you have high blood pressure.

The Well Baby Well Child Program consists of a multidisciplinary team consisting of your Primary Care Provider (Physician or Nurse Practitioner), Registered Nurse, Registered Practical Nurse, and/or a Clinical Assistant.

The goal of the program is to monitor the baby’s growth and development according to the Rourke Baby Record Evidence Based Infant/Child Health Maintenance Schedule and the Nipissing Developmental Screen. Our team will take the time to listen to your concerns and answer your questions in a supportive, family-friendly environment.

During your visit our team provides:

  • Physical and Developmental Assessments
  • Immunization
  • Education to Parent/Caregiver

Screenings and assessments are all based on Evidenced Best Practice Based Guidelines.
