The Powassan and Area Family Health Team would like to provide patients and their families with an opportunity to express comments, concerns and suggestions. We strive to improve the care and service that we provide to our patients, and we value your feedback.

This process is intended to be for general comments and suggestions about Powassan FHT's services, and should not pertain to your own healthcare. If you have questions about your health, please contact your clinic directly.

PAAFHT puts its patients first and therefore takes any complaint very seriously.
If you would like to file a complaint, please complete this form and submit it to the attention of the Executive Director, Anna Gibson-Olajos. Any complaint will receive immediate attention. You will be contacted within five (5) working days of receiving the complaint and investigation will be completed within forty (40) working days of its initiation.
